Child Care Facilities

All children in North Carolina must be vaccinated against certain diseases. Immunization records are checked when a child enters a child care facility.

Find out what vaccines children need before attending a child care and view resources for child care facilities.

Annual Immunization Reports

Every year, all child care facilities are required to report the immunization status of their students. Reporting forms for child care facilities can be found here.
